Terms of Service
These Terms of Service (hereinafter referred to as "these Terms") define the conditions applicable to all customers (hereinafter referred to as "Users") who use the services provided by Pond Reve LLC (hereinafter referred to as "this Service"). Users shall use this Service after agreeing to these Terms.
Article 1 (Scope of Application)
These Terms aim to define the rights and obligations between our company and Users regarding the house call reception service provided by our company, and apply to all Users.
Article 2 (Definitions)
1."Our Company" refers to Pond Reve LLC.
2."Medical Institution" refers to a medical corporation or doctor partnered with our company.
3."House Call" refers to a service where a doctor visits the User's location to provide medical examination and treatment.
Article 3 (Service Content)
1.This Service is a house call reception service where, upon application by the User, a doctor visits the User's location to provide medical examination and treatment.
2.Medical treatment is provided by a partnered doctor, and our company is not involved in the medical act itself.
3.This Service is generally intended for mild symptoms that do not require urgent care. For highly urgent symptoms, please use an ambulance.
Article 4 (User Eligibility)
This Service can be used by those who meet the following conditions:
1.Able to use the service responsibly.
2.Minors (under 18 years old) require parental consent.
3.Overseas travelers or temporary visitors who are not enrolled in health insurance in Japan agree to pay medical expenses at their own expense.
Article 5 (Fees and Payment)
1.Users shall pay the medical examination fees, transportation fees, treatment fees, and other costs associated with the house call.
2.Payment shall be made by the payment method designated by our company (credit card, online payment, etc.).
3.The cancellation policy shall be as stipulated on the service website.
Article 6 (Handling of Personal Information)
Users' personal information will be appropriately managed in accordance with our company's privacy policy. Users agree that our company will acquire and use information voluntarily entered or provided by Users and other information stipulated in the privacy policy when using this Service.
Article 7 (Prohibited Acts)
Users shall not engage in the following acts:
1.Acts that violate these Terms.
2.Acts that violate laws or ordinances, or acts related to criminal activities.
3.Fraudulent or threatening acts against our company or third parties.
4.Acts contrary to public order and morals.
5.Acts related to anti-social activities, or providing benefits to anti-social forces.
6.Acts that infringe upon the intellectual property rights, portrait rights, privacy rights, honor, or other rights or interests of our company or third parties.
7.Slandering or defaming our company or third parties.
8.Unauthorized access to our company's network or system, or attempting unauthorized access.
9.Providing false information.
10.Verbal abuse, violence, or nuisance behavior towards doctors or staff.
11.Unauthorized cancellation or obstruction of medical treatment.
12.Other acts deemed inappropriate by our company.
Article 8 (Disclaimer)
1.All medical acts are performed under the responsibility of the doctor, and our company bears no responsibility whatsoever for the content or results of the medical treatment.
2.Our company is not responsible for delays or interruptions in service due to natural disasters, traffic conditions, communication failures, etc.
Article 9 (Suspension/Interruption of Service)
Our company may suspend or interrupt this Service in the following cases:
1.Insufficient medical resources, force majeure such as disasters or accidents.
2.When system maintenance or inspection is required.
3.Other cases where our company deems it necessary.
Article 10 (Governing Law and Jurisdiction)
The governing law for these Terms and the use of this Service shall be Japanese law. All disputes arising from or related to these Terms or the use of this Service shall be subject to the exclusive agreement jurisdiction of the Tokyo District Court as the court of first instance.
